SLAC Cyber Security Exception Form

Cyber Security Exception Form is required for any SLAC non-standard systems setup e.g., systems not to be managed, not being updated or patched centrally, not having multi-factor authentication on system logon, etc. 

Please be prepared to discuss the following information: 

• Review the SLAC Minimum Security requirements.
• Background information and the reason prompting the exception request. 
• Risk to the lab if this exception is not approved
• If science is going to be halted due to not having and exception, please bring this to our attention.
